What is the difference between assisted living and a nursing home in Kentucky?

A Kentucky nursing facility provides 24-hour licensed nursing care under OIG's long-term-care licensing rules and, when Medicare- or Medicaid-certified, is inspected and star-rated by CMS; an assisted living community provides help with daily living in a residential setting and is not certified or rated by CMS.

KRS 194A.707(8) goes so far as to require OIG's assisted living reviewers to understand that assisted living “is not subject to the rules and regulations of the Centers for Medicare and Medicaid Services.” Nursing homes are; that is why our nursing home roster can show CMS ratings and our assisted living roster cannot.

When does a Louisville parent need a nursing home instead of assisted living?

A parent needs a nursing facility when they need daily skilled nursing or rehabilitation, cannot move from place to place even with a device and help, need two-person transfers around the clock, or have medical needs no assisted living license can meet.

Kentucky's assisted living residents must be ambulatory unless temporarily not, under a broad definition that includes wheelchair users. Short-term rehab after a hospital stay usually happens in a nursing facility's Medicare-certified beds and ends with a return home or to assisted living.

How do costs compare between assisted living and nursing homes in Kentucky?

CareScout's 2025 Kentucky medians are about $5,290 a month for assisted living and about $9,718 a month for a semi-private nursing home room ($11,254 private), and Kentucky Medicaid pays for nursing home care for eligible residents but has no waiver that pays for assisted living.

That Medicaid asymmetry is the most important financial fact in this comparison. A resident who will outlive their savings in assisted living may eventually need a Medicaid nursing facility bed even if their care needs have not changed much.

How should Louisville families compare nursing homes?

Start with CMS's overall, health inspection, staffing and quality ratings, then read the latest inspection's deficiency list, staffing hours per resident day and turnover, all of which CMS publishes and our facility pages reproduce for each certified Louisville-area nursing home.

Visit at shift change and on a weekend; CMS reports weekend nurse staffing separately because it is often thinner.
