What is a nursing home in Kentucky?

A Kentucky nursing home, or nursing facility, provides 24-hour licensed nursing care and rehabilitation under an OIG long-term-care license, and most are also certified by CMS for Medicare and Medicaid, which brings federal inspections and Care Compare star ratings.

OIG's long-term-care roster splits beds by type: NF (Medicaid nursing facility), NH (state-licensed nursing home), ICF, Alzheimer's and personal care. A few Louisville facilities hold only state NH beds and are not CMS-rated.

CareScout's 2025 Kentucky medians are $116,618 a year (about $9,718 a month) for a semi-private room and $135,050 a year (about $11,254 a month) for a private room, both up more than 10% from 2024.

Medicare covers limited skilled stays after a qualifying inpatient admission; long stays are private pay, long-term care insurance or Kentucky Medicaid, which requires resources under $2,000 for one person.

Watch for the Special Focus Facility designation, which CMS gives nursing homes with persistent serious problems.

What should Louisville families ask a nursing home first?
